Eligibility
Any woman who is 40 or over and is currently underinsured or uninsured by a health plan or Medi-Cal/Medicare may be eligible for screening and detection services from CDP. Underinsured means:

  • Unable to pay copayment or deductible
  • Unable to pay Medi-Cal share of cost
  • Limited or restricted coverage

    In addition, all patients must have a houshold income at or below 200 percent of the poverty guideline. “Household income” includes the income of all family members living together. Poverty guideline level incomes are adjusted on an annual basis. The following Income Eligibility chart is effective from April 1, 2004 through March 31, 2005. “Gross income” means income before taxes and other deductions.

Application
1. Call 1-800-511-2300 to see if you are eligible. You will be asked about your age, income, and health coverage to see if you can be examined in this program. Services at this phone number are available in English, Spanish, Mandarin, Cantonese, Korean and Vietnamese.

2. If eligible, you will receive a list of CDP doctors and clinics in your area. Use this list to call to make an appointment for a Clinical Breast Examination.

3. Go to your appointment. You will receive an identification card (similar to an insurance card) from a staff person. Bring this card to every appointment, as this card lets the doctor’s office staff know that CDP will pay for your appointments and tests.

4. Keep all your appointments for examinations or tests recommended by the doctor until you know you do not have breast cancer.

5. Examine your breasts every month and have your breasts examined yearly by a medical provider, along with a mammogram.

The CDP Gold Country Regional Partnership is a coalition of people and organizations from 13 counties in Northern California working together to save women’s lives through early detection and screening services. For information, call the Health Education Council at (916) 556-3344.
